PEDIATRIC CLINICS AMSTERDAM Cardioprotective interventions for cancer patients receiving anthracyclines: a Cochrane systematic review

چکیده
Anthracyclines have gained widespread use in the treatment of numerous malignancies in both adults and children. Unfortunately, their use is limited by the occurrence of cardiac damage. The precise mechanism underlying this is still not fully understood, but a major role has been ascribed to free radical generation by anthracycline-iron complexes. The heart is particularly vulnerable to free radical injury because protective enzymes such as superoxide dismutase are present at a lower level in heart tissue than in other tissues. The cardiac damage can become manifest as either clinical heart failure or asymptomatic cardiac dysfunction, which may not only develop during anthracycline therapy, but also years after the cessation of treatment. The risk of developing heart failure therefore remains a lifelong threat, especially to children who have a long life-expectancy after a successful antineoplastic treatment. In the literature the frequency of clinical heart failure has been reported to be as high as 16% and that of asymptomatic cardiac dysfunction to be more than 57%. Several risk factors, such as a higher cumulative anthracycline dose, cardiac radiotherapy and female gender, have been identified, although not conclusively in all studies. Background: Stress and anxiety in cancer patients are caused by disease diagnosis, unfamiliar experiences, and therapy-related problems. In addition to the short duration of radiotherapy, receiving and understanding of the information about this treatment could be difficult for patients due to anxiety, fatigue, and mental pressure. متن کاملThe Effect of Lifestyle Intervention on Pregnancy and Birth Outcomes on Obese Infertile Women: A Systematic Review and Meta-Analysis
Background: Obesity has been associated to negative effects on natural fertility and a worse prognosis when assisted reproduction techniques are performed. Patients attending for fertility treatments are often advised to optimize their weights to improve outcomes. There are doubts about how effective are weight-loss interventions for improving fertility in women who are overweight or obese. Mat...
متن کاملThe use of liposomal anthracycline analogues for childhood malignancies: A systematic review.
In an effort to prevent or reduce anthracycline-induced cardiotoxicity, liposomal anthracyclines have been developed. The objective of this systematic review was to summarise all available evidence on the benefits and risks of liposomal anthracyclines in children with cancer. We searched databases (MEDLINE (1966-September 2009), EMBASE (1980-September 2009) and CENTRAL (The Cochrane Library, is...
متن کاملPrognostic Value of BAALC Expression in Pediatric Acute Myeloid Leukemia: A Systematic Review
Background: Acute myeloid leukemia (AML) is a malignant disorder involving blood cells, characterized by obstructed or distorted differentiation of hematopoietic stem cells. In cytogenetically normal AML (CN AML), molecular abnormalities in NPM, FLT3, CEBPA, and BAALC genes are observed. Initially, high BAALC (Brain and Acute Leukemia Cytoplasmic gene) expression was introduced in a study on AM...
